We provide residential services and advocacy in areas such as competency dockets, navigating the justice system or parole, waiver assessments and referrals, behavioral therapy and support, and mental health services, with a psychiatric nurse practitioner on-site.

House was founded in 2006 by Thakane Mitchell, RN, BSN, during her part-time
work at Denver Health and at Brighton Gardens, where she served as a charge
nurse in the skilled nursing unit. With a deep commitment to helping others, Thakane
transitioned to full-time work at Jacob House in 2006, where she worked
alongside her husband, Dr. David Mitchell, PhD, to provide essential services
to individuals with intellectual disabilities and those in need.
Tragically,
in 2011, Thakane was diagnosed with colon cancer. As her illness progressed,
Dr. Mitchell demonstrated extraordinary dedication by resigning from his job to
care for Thakane and their two young children. During this challenging time, he
took on the management of Jacob House, which initially served only two clients.
His personal sacrifices and unwavering commitment were instrumental to the
organization's growth and continued success.
Thakane
Mitchell, a crucial figure in the creation and mission of Jacob House, passed
away on July 29, 2011. Her absence left a profound void, yet her legacy
continues to inspire the staff and community to uphold her vision. The mission
of Jacob House remains a testament to her dedication and enduring influence.
Today,
Jacob House stands as a beacon of hope, offering a comprehensive range of
services as a residential agency. It plays a vital role in supporting
individuals with disabilities, advocating for their independence, and helping
them develop essential lifelong skills. The services provided include a variety
of residential supports such as host home services, family caregiver support,
respite care, independent living services, community resources, advocacy, and
transportation assistance. Jacob House strives to empower those it serves,
enabling them to lead fulfilling lives with a robust network of support and
care.
