Our Host Home Residential program provides a personalized, supportive, and homelike environment for individuals with disabilities, offering supportive living arrangements designed to promote physical and mental well-being, as well as independence.

Host Homes provides personalized support tailored to each person’s unique needs. This can include assistance with daily
living skills, socialization, medical care, financial management, and participation in community activities, all customized to the individual’s specific circumstances and preferences, fostering independence and self-reliance. Independent contractors can partner with Jacob House to offer a holistic, family-like experience to those in our care.
